Ordinanceby the Minister of Economy and Labourof 1 October 2004on format for delivery verification certificate and the method for registration of issued certificatesPursuant to Article 24.4. of the Act of 29 November 2000 on foreign trade in goods, technologies and services of strategic importance to the security of the State and to maintaining international peace and security (Journal of Laws of 2004 No 229, item 2315), the following is hereby adopted: § 1. A format for delivery verification certificate is hereby defined and contained in the Annex herein. § 2. 1. Register of delivery verification certificates issued shall be maintained by the head of a competent customs office in the form of a register book. 2. The head of a competent customs office shall appoint a person authorised to maintain the register of delivery verification certificates issued. § 3. 1. Entries in the register book shall be based on data from delivery verification certificates. 2. Register book shall be kept in accordance with the provisions on documentation classification and qualification for register purposes. § 4. 1. Each entry to the register book shall be designated with a number based on the order of entries, and have an entry date and a signature of the person authorised to maintain the register. 2. Entry to the register book shall be permanent and clear, it cannot be erased or otherwise removed. 3. Corrections shall be made clearly. 4. The person authorised to maintain the register shall make a note in the entry on any corrections made, stating the substance of the correction and reasons for the latter, indicating in particular the item subject to correction, and providing a signature. §5. Register of the delivery verification certificates issued shall contain the following information: 1) issuing date of the delivery verification certificate; 2) name of the body that issued the delivery verification certificate; 3) importer’s name, seat and address; 4) exporter’s name, seat and address; 5) name and description of the commodity or technology; 6) commodity control number; 7) quantity and value of commodities or technologies; 8) description of ultimate use of the commodity or technology; 9) SAD document number; 10) bill of lading number, CMR number or a number of any other document attesting the commodity or technology delivery; 11) T5 Control Card number required under international law. § 6 The Regulation by the Minister of Economy of 13 May 2002 on format for delivery verification certificate and the method for registration of issued certificates (Journal of Laws No 59, item 545) is hereby repealed. § 7. This Regulation shall enter into force after 14 days following its announcement.
